TLC Terms and Conditions

The Availment Process

Simply contact VFA H.R. POC for assistance. Present invoice and hospital phone number for verification.

Philhealth and HMO must be maximized first before VFA’s TLC Benefits can be applied.

We reserve the right to withdraw or amend the Service and Terms of Service without notice.

Membership Eligibility

Employees at VFA should commit to a minimum tenure of one (1) year, serving clients at or above that standard.

Only for VFA’s full time independent contractors

First Dependent can be enrolled in your second year in VFA

Second dependent can be enrolled in your third year in VFA

  1. Principal member: Any person at least 18 years old up to 59 years of age
  2. Qualified Dependent Member:
    a. For married principal member:
       I. Legal spouse up to 59 years of age
       II. Legitimate and/or adopted children 6 months old and up to 21 years of age who             are not gainfully employed or unmarried
    b. For single principal member:
       I. Parents up to 59 years of age
       II. Brothers and sisters 6 months old and up to 21 years of age
    c. For single parent principal member:
       I. Children 6 months and up to 21 years of age
  • The choice of enrolling dependents must follow a hierarchy. This means that for a married principal member, the spouse must be enrolled first followed by the eldest child, second child, and so on.
  • For a single principal member, the parents must be enrolled first followed by the eldest brother/sister, and so on.
  • If the principal member is married, his/her brothers or sisters are no longer eligible as qualified dependents unless expressly provided for this agreement.

Membership Benefits and Privileges

Covers all in-patient services.
In-patient – A person who has been admitted to the hospital as a registered bed patient and is receiving services under the direction of a licensed physician.

No emergency care benefits

Non-transferrable

No out-patient services

No dental service care

Php 50,000 limit a year. Not per illness. This is accumulated

No preventive health care services
